A Risk Score Is a Snapshot. Health Is a Trajectory: 10P-Health in Action
A landmark Nature study offers a glimpse into what I call the 21st Century Health & Wellness Paradigm—10P-Health. It introduces ALADYNOULLI, a Bayesian framework that integrates longitudinal health records, age and polygenic risk to model how an individual’s disease risks evolve over time.
Across three independent biobanks, the study included more than 683,000 individuals, 348 diseases and up to 52 years of follow-up. The model recovered 21 replicable disease signatures with 80% median preservation across cohorts, revealing that people with the same diagnosis may reach it through different biological pathways.
ALADYNOULLI outperformed the Pooled Cohort Equation, PREVENT and Gail clinical risk calculators at both 1- and 10-year horizons. Its signature-based genomic analysis identified 151 genome-wide significant loci, including cardiovascular associations missed by conventional single-disease analyses.
The deeper advance is putting time back into 10P-Health. A conventional score asks how high the risk is; a trajectory asks how it is changing, what may be driving it and when intervention might still make a difference. Yet prediction is not prevention: this observational modelling study used biobank and EHR data, and improved patient outcomes or real-world clinical utility have not been demonstrated. Turning a rising trajectory into better health will require prospective validation, causal evidence and integration into clinical decisions.
